Practice starts Apr 6. You may register your athlete throughout the season. Complete the forms below to complete the season.

Costs

Below are the required and optional costs for each athlete.

90% goes to covering to cost of meet entry fees.

Optional costs

If you would like your athlete to compete at State, there is an additional $49-$79 charged by USATF. Range is based on how many events your athlete does. If your athlete qualifies for Regionals, or Nationals, there is a $15-$60 entry fee for each.

Optional gear will be for sale in March.

Code of Conduct and Minor Athlete Policy

CODE OF CONDUCT

All members agree to:

Show respect to coaches, athletes, and officials
Compete with integrity
Follow SafeSport guidelines
Avoid bullying, harassment, or abuse
Maintain sportsmanship at all times

Violations may result in suspension or removal.

MINOR ATHLETE SAFETY POLICY

No 1-on-1 unsupervised interactions between adult and minor
Communication must include parent/guardian when appropriate
No private transportation without consent
No inappropriate physical contact

Club follows US SafeSport guidelines.

Membership Agreement & Liability Waiver

This Agreement is entered into between Canby Youth Track (“Club”) and the undersigned athlete or parent/guardian (“Member”).

MEMBERSHIP
Member agrees to participate in Club activities including practices, competitions, and related events.
ELIGIBILITY
Member agrees to maintain active USA Track & Field (USATF) membership if competing in the post season at State, Regionals, or Nationals.
FEES
Member agrees to pay all applicable fees. Fees are non-refundable unless otherwise stated.
RULES & COMPLIANCE
Member agrees to:

Follow all USATF rules and regulations
Comply with SafeSport policies
Follow all instructions from coaches and staff

HEALTH & MEDICAL
Member certifies they are physically able to participate and will disclose any relevant medical conditions.
TERMINATION
Club reserves the right to suspend or terminate membership for:

Unsafe behavior
Violation of policies
Non-payment

GOVERNING LAW
This Agreement shall be governed by the laws of the State of Oregon.

In consideration of being allowed to participate in activities with Canby Youth Track, I agree:

ASSUMPTION OF RISK
I understand that track & field activities involve risks including serious injury or death.
RELEASE
I release and hold harmless:

Canby Youth Track
Coaches, volunteers, staff
Facility providers
USA Track & Field (USATF)
from any liability arising from participation.

INDEMNIFICATION
I agree to indemnify and defend the above parties from any claims arising from my participation.
MEDICAL TREATMENT
I authorize emergency medical treatment if necessary.
MEDIA RELEASE
I grant permission to use my own or my athlete's image, likeness, or performance on the team website or social media account.
ACKNOWLEDGMENT
I have read and understand this agreement and sign voluntarily.
